Important Notice
We are pleased to announce that the Office of Professional Regulation, including all licensing boards and advisory professions, will upgrade to a new licensing computer system beginning the week of February 9th. The Office of Professional Regulation took the following actions in the month of according to Secretary of State Deborah Markowitz:

LICENSEE:Jeffrey G. Healey, West Dover, VT
LICENSE TYPE:Real Estate Salesperson (Applicant)
VIOLATION: According to Findings of Fact, Conclusions of Law and Order, Healey was preliminarily denied a license based upon his conviction of a felony.
SANCTION: On March 3, 2003 the Real Estate Commission granted Healey a conditioned license.

LICENSEE: Richard Barker, Brookline
LICENSE TYPE: Pharmacist
VIOLATION: According to Stipulation and Consent Order approved by the Board of Pharmacy, Barker failed to keep a record of all regulated drugs dispensed by him and removed prescription drugs without authority from his places of employment. 
SANCTION: On March 5, 2003, the Board suspended the license of the Barker.

LICENSEE: Bruce McLaughry, Norwich, VT
LICENSE TYPE: Real Estate Broker
VIOLATION: According to Stipulation and Consent Order approved by the Real Estate Commission an employee of McLaughry=s was disciplined for improperly handling a limited agency disclosure situation.  As principle Broker McLaughry is responsible for the professional conduct of his employees.
SANCTION: On March 3, 2003,the Vermont Real Estate Commission issued a reprimand to McLaughry.

LICENSEE: Dana Pictou, E Calais, VT
LICENSE TYPE: Drug and Alcohol Abuse Counselor
VIOLATION:   According to a Stipulation and Consent Order approved by the Administrative Law Officer, Pictou erroneously billed Medicaid for services not performed and engaged in a dual relationship with client.
SANCTION: On March 03, 2003 the Administrative Law Officer conditioned respondent=s license for a minimum period of two (2) years. 

LICENSEE: Kevin Graf, Lyme, NH
LICENSE TYPE: Tatooist
VIOLATION:.  According to the Stipulation and Consent Order approved by the Administrative Law Officer, Graf was convicted in the State of New Hampshire for tattooing a minor without the parents= consent.
SANCTION: On March 03, 2003, the Administrative Law Officer conditioned Graf=s license for thirty-six (36) months.

LICENSEE: Shonda Clark, St Albans, VT
LICENSE TYPE: Nursing Assistant
VIOLATION: According to Default Order approved by the Board of Nursing, Clark failed to answer charges that she left a  facility without providing proper notice and providing improper incontinent care to a resident.
SANCTION: On March 10, 2003, the Board  revoked her license.

LICENSEE: Samantha McCray, Hoosick Falls, NY
LICENSE TYPE: Nursing Assistant
VIOLATION: According to Default Order approved by the Board of Nursing, McCray failed to answer charges that she left a facility without providing proper notice.
SANCTION: On March 10, 2003, the  Board revoked her license.

LICENSEE: Nancy Contois, South Burlington, VT
LICENSE TYPE: Registered Nurse
VIOLATION: According to Stipulation and Consent Order approved by the Board of Nursing, Contois violated the conditions of a previous Stipulation and Consent Order.
SANCTION: On March 12, 2003, the  Board suspended her license.

LICENSEE: Heidi King, East Arlington, VT
LICENSE TYPE: Nursing Assistant
VIOLATION: According to Stipulation Consent Order approved by the Board of Nursing, King admitted she had recent criminal convictions and was addicted to heroin.
SANCTION: On March 12, 2003, the Board conditioned King=s license for a minimum period of three (3) years.

LICENSEE: Tammy Brigham, Hyde Park, VT
LICENSE TYPE: Nursing Assistant
VIOLATION: According to Stipulation and Consent Order approved by the Board of Nursing, Brigham admitted she documented a time sheet that she=d been at patient=s home and it was not accurate.
SANCTION: On March 12, 2003, the Board reprimanded Brigham.

LICENSEE: Eric J. Friedman, Barre, VT
LICENSE TYPE: Psychologist Examiner
VIOLATION: According to Stipulation and Consent Order approved by the Board of Psychological Examiners, Friedman did not provide clients with a written disclosure form.
SANCTION: On March 07, 2003, the Board issued a warning to Friedman.

LICENSEE: Jill Bessette, Burlington, VT
LICENSE TYPE: Nursing Assistant
VIOLATION: According to Default Order approved by the Board of Nursing , Bessette failed to answer the charges against her that she failed  to disclose felony convictions on her application.
SANCTION: On March 20, 2003, the  Board revoked Bessette=s license.

LICENSEE: Laura Crosby, North Adams, MA
LICENSE TYPE: Nursing Assistant
VIOLATION: According to Findings of Fact, Conclusions of Law and Order, Crosby was disciplined in Massachusetts for neglecting a resident.
SANCTION: On March 20, 2003, the  Board indefinitely suspended Crosby=s license.

LICENSEE: Carlena Jo Budwick, No Hudson, NY
LICENSE TYPE: Licensed Practical Nurse
VIOLATION: According to Findings of Fact, Conclusions of Law and Order, Budwick was convicted of obtaining prescription medications for herself and spouse.
SANCTION: On March 20, 2003, the  Board revoked Budwick=s license.

LICENSEE: Crystal R. Dubuque, Grand Isle, VT
LICENSE TYPE: Cosmetologist
VIOLATION: According to Stipulation and Consent Order, Dubuque was convicted of possession of a controlled substance in New York State.
SANCTION: On March 24, 2003, the Vermont  Board of Barbers and Cosmetologists has indefinitely conditioned Dubuque=s license.

LICENSEE: Ingrid E. Halligan, Danby, VT
LICENSE TYPE: Cosmetologist
VIOLATION: According to Stipulation and Consent Order, Halligan allowed an unlicensed person to practice cosmetology at her shop.
SANCTION: On March 24, 2003, the Vermont Board of Barbers and Cosmetologists  reprimanded Halligan=s shop license and conditioned the personal license of Halligan.
